Job Description Job Description Salary: 50K to 60K DOE PURPOSE Coordinate, access, and provide Community Support services and/or any other services in accordance with wraparound philosophy and in the least restrictive setting possible to youth and adults who qualify for community support and/or Developmental Therapies services; act as a liaison between member, family, providers, and the community; develop, facilitate and implement interventions through the members team of providers and supports according to an individualized treatment plan; maintain agency records and documentation; respond to crisis. ESSENTIAL DUTIES AND RESPONSIBILITIES Under general supervision, coordinates the intake and eligibility process; assists the individual using services in obtaining services, performs functional assessments and social histories; convenes an Interdisciplinary Team; facilitates the development of the Individual Service Plan; establishes meaningful goals, coordinates and monitors services in compliance with Administrative Rules, Services Management Plan; and other applicable policies and procedures, measures progress toward goals. maintains case files, correspondence, and all other required data; mediates conflicts and advocates for individuals using services. Knowledge of: Community Support Team policies and procedures Clinical diagnosis and treatments Human services and MH/DD/SAS network Principles of choice, empowerment, and community integration Disability related income sources State Services and Definitions as outlined in MH/DD/SAS Diagnostic and statistical manual Basic office practices and procedures including record keeping and filing systems English language to include grammar, spelling, sentence structure, and vocabulary; and proofreading skills General math aptitude Ability to: Follow a professional code of ethics including confidentiality, individual rights, and professional and legal issues in providing services Maintain confidentiality of protected information Understand and apply policies, rules and regulations Exercise good judgment in analyzing situations, answering questions, and interpreting policies Collect data, keep accurate and organized records, and ensure timely preparation of reports Organize and prioritize multiple tasks Professionally present and represent MH/DD/SAS services that are courteous and responsive Respond effectively during crisis situations Work independently on projects and routine tasks Follow oral and written instructions Establish and maintain effective working relationships with individuals using services and their support network, co-workers, supervisors, agencies and members of the public Type accurately and use a computer for the purposes of data entry, spreadsheets, word processing, email and the Internet Handle moderate to high levels of stress, meet established deadlines and solve problems appropriate to the position Communicate effectively both verbally and in writing o Have clarity of speech and hearing, which permits effective communication Have sufficient manual dexterity to make handwritten notations and which permits moderate use of a keyboard Have sufficient vision, which permits moderate production and review of a wide variety of materials both in electronic and hardcopy forms Lift, move, push and pull up to 10 pounds Sit for extended time periods, bend, and reach Have sufficient personal mobility to complete fieldwork at various facilities Work flexible schedule and additional time as needed SPECIFIC DUTIES To be performed satisfactorily with or without reasonable accommodation, the following duties are normal for this position. Performs intake duties and explains agency services to applicants Processes applications for services by conducting intake interviews, completing social histories, obtaining necessary assessments and determining eligibility Determines legal settlement by verifying information, contacting providers, county and family members Identifies alternative funding and assists the individual using services in accessing other financial supports Provides information regarding available community resources and its natural supports Convenes Admission Team and coordinates development of Individual Service Plan Assists individuals, their families, and their guardians in maximizing their abilities for self-direction by empowering them to achieve the greatest possible degrees of independence Mediates conflicts between providers, family members, individuals using services, legal representatives, and/or the agency relative to the needs of the individual, as identified in the Individual Service Plan Works with the Interdisciplinary Team to identify needs and establish meaningful goals Requests funding appropriate to identified needs Coordinates the implementation and monitoring of the Individual Service Plan Monitors progress toward goals using required quarterly reports Initiates discharge planning process to transfer/close cases when services are no longer needed or desired Ensures compliance with regulatory requirements for incident reporting Maintains case files and resource files according to established standards Coordinates Community Support Team Services to established State Definitions As a mandatory reporter, maintains approved training on child and dependent adult abuse Participates in MH/DD/SAS Services, agency, provider and statewide committee meetings Performs all work duties and activities in accordance with EQCS policies, procedures, and safety practices MINIMUM EDUCATION. TRAINING AND EXPERIENCE A Bachelor's degree with 4 years of experience working in a human service-related field under a QP in some capacity or 30 semester hours in a human services field (including, but not limited to, psychology, social work, mental health counseling, marriage and family therapy, nursing, education, occupational therapy, and recreational therapy) and at least one year of experience in the delivery of services to the population groups that the person is hired as a case manager/QMHP to serve. An individual who holds a license, provisional license, certificate, registration or permit issued by the governing board regulating a human service profession, except a registered nurse who is licensed to practice in the State of North Carolina by the North Carolina Board of Nursing who also has four years of full-time accumulated experience in MH/DD/SA with the population served, or a graduate of a college or university with a Masters degree in a human service field and has one year of full-time, post-graduate degree accumulated MH/DD/SA experience with the population served, or a substance abuse professional who has one year of full-time, post-graduate degree accumulated supervised experience in alcoholism and drug abuse counseling. CONDITIONS OF EMPLOYMENT Satisfactory Background Investigation Valid NC Drivers License and proof of personal vehicle insurance coverage CPR/First Aid Certification & Bloodborne Pathogens Training Non-Violent Crisis Intervention Training Ability to successfully complete service-specific training
