Healthy Communities Specialist (Lifestyle Coach) - Bilingual

Job Description:

Hours: 8:00 - 5:00 Monday - Friday, 40 hours per week (occasional nights and weekends)

Hiring Range: $18.98 - $24.00 /hour

Funding Period is guaranteed until the end of FY 2022 and will be renewed based on grant continuation

The Healthy Communities Specialist (Lifestyle Coach) is responsible for the planning, implementation and tracking of the CDC "Prevent T2" or "Prevent el T2" Lifestyle Class for the Minority Diabetes Prevention Program (MDPP) grant as well as the Healthy Communities program. Healthy Communities enables county and district health departments to implement community-based interventions that address poor nutrition, physical inactivity, tobacco use, violence and unintentional injury. This position is funded by the Division of Public Health for the Office of Minority Health and Health Disparities (NC OMHHD) and the NC Preventive Health and Health Services (PHHS ) Block Grant. Cabarrus Health Alliance is the lead agency for MDPP's 11-county region and a Healthy Communities coordinating agent, so frequent travel is required.

Primary Responsibilities:

  • Work with regional partners to implement "Prevent T2" or "Prevent el T2" Lifestyle classes
  • Provide support and guidance to Healthy Communities' partners in the implementation of the NC Healthy Food Retail Designation and/or Healthy Food Policies/Guidelines; MDPP participants in the lifestyle program and implementation of the CDC's standard curriculum
  • Deliver the program in a way that increases the capacity of DPP participants and Healthy Communities' partners to make and sustain positive lifestyle and environmental changes
  • Enter participant and program data into the MDPP database on a weekly basis
  • Maintain a record of Healthy Communities action plan updates and other activities on a monthly basis
  • Assist with other department programs (i.e. Lifestyle Medicine, Culinary innovations, WIC program outreach, Employee wellness ) as needed

Education/Experience:

  • Graduation from a four year college or university with a degree in health education or graduation from a four year college or university with considerable experience in public health education
  • Must have experience working with youth and adults in community health education
  • Ability to work well with other people of diverse racial and cultural backgrounds

License: Valid NC driver's license

Skills & Abilities:

  • Proficient in English and Spanish
  • Able to maintain effective working relationships and build trust with patients in minority communities
  • Must be self motivated, detail oriented, flexible, innovative and possess strong problem solving, organizational and administrative skills

Knowledge:

  • Considerable knowledge of diabetes practices, chronic disease prevention and evidence-based programming preferred
