Agape Care Group is seeking a compassionate and dedicated Registered Nurse (RN) to join our team, providing hospice and palliative care to patients across the Southeast. In this vital role, you will work closely with patients and their families to offer end-of-life care that respects the dignity of each individual and supports our mission of delivering personalized and high-quality comfort. Our ideal candidate is a patient advocate who is committed to creating meaningful moments and enhancing the quality of life for our patients during their most significant times. Assess patient conditions during visits and adjust care plans accordingly Administer medications and treatments as prescribed by the attending physician Educate patients and their families on proper home health care strategies and procedures Coordinate with doctors, multidisciplinary team members, and healthcare professionals to provide continuous care Document patient care services by charting in patient and department records Ensure compliance with all laws, regulations, and protocols in hospice and palliative care Excellent interpersonal and communication skills Proficient in using electronic health records and medical software Strong clinical skills in patient care and assessment Ability to handle emotional and high-stress situations Compassionate and strong patient advocacy Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current RN license in the state of employment CPR certified Preferred: Certified Hospice and Palliative Nurse (CHPN)
