Job Description:
As a Licensed Practical Nurse (LPN) at Liberty Healthcare, you
will be a crucial member of our team dedicated to providing

patient care through the nursing process of assessment, planning,
implementation, and evaluation. This position demands a

Monitor, record, and report symptoms or changes in patients'
conditions while maintaining accurate and detailed patient records.
Administer nursing care to ill, injured, convalescent, or disabled
patients. Advise patients on health maintenance and disease
prevention or provide case management. Implement physician-directed
care plans, administer medications, and oversee patient progress.
Assist in the collection of patient data such as health history,
reports, and results for clinical use. Communicate regularly with
patients and their families, offering detailed documentation of
treatment and recovery. Prepare and maintain rooms with necessary
supplies and equipment for treatment. Participate in educational
opportunities to update knowledge and skills, adhering to state
regulations and organizational requirements. Strong clinical skills
including medication administration, wound care, and life support
Excellent teamwork and multi-tasking abilities Effective
communication and interpersonal skills Proficient in using
healthcare software and systems Medical Insurance Dental Insurance
Vision Insurance Life Insurance Current LPN license in the state of
practice Basic Life Support (BLS) certification, Advanced Cardiac
Life Support (ACLS) is a plus
