families and patients about end-of-life care and expected decline
Description: This position is a field Hospice role requiring travel
to patient homes and Long Term Care facilities. The candidate will
work remotely to start the day and will transition to field visits.
Attendance at biweekly interdisciplinary team meetings is required.
The typical schedule falls within the hours of 0830 to 1700, Monday
through Friday, with optional night/weekend shifts. An average
caseload consists of 14-15 patients, with ideally 5-6 visits per
day. The role provides autonomy in managing one’s schedule and
travel to patient locations. Mileage reimbursement is provided at
the current IRS rate, calculated daily through a mobile system.
Required patient care includes end-of-life care for
Hospice-eligible patients. The interdisciplinary team will conduct
case conferences to evaluate patient load every other Wednesday.
Support is available through peer RNs, nurse educators, clinical
supervisors, triage support, social workers, case managers, and
medical team members. Technology used includes an EMR system and a
tablet for charting purposes. Orientation includes a minimum of one
week of shadowing and training with other caregivers. Approval for
time off is required through the contract. Scrub color for the
position is navy.
