The average number of patients per day is 8, with a minimum of 9 points that must be met daily. You will primarily work with adult patients with various chronic conditions. Common diagnoses include chronic medications, cardiac issues, heart failure, COPD, and wounds. Triage functions will serve as weekend leadership. Support will be available within the department from other RNs. You will be required to use EMR software for charting. Experience with the Homecare Homebase system is essential. Knowledge of the Home Health Oasis assessment tool is also necessary. Preferred experience includes using IV Pumps and wound vacs. You may be asked to float into nearby counties, but this is not common. The position entails working holidays as scheduled. Requests for time off will require manager approval. Scrub color for this position is navy.
